EU AND NI PET TRAVEL FORM  

 
Please complete this form in advance of your appointment with an Official Veterinarian to issue your pet(s) with an Animal Health Certificate (AHC).  
 
We will need you to bring on the day of your appointment: 
 
the pets you intend to travel with  
proof of their up-to-date rabies vaccination 
proof and date of identification of the pet(s) (e.g. microchip, tattoo). 
 
If you have a Pet Passport that was issued before 31/12/2020, this will suffice if their Rabies vaccine is in date. 
 
If you do not have a Pet Passport, please bring your vaccination card that shows the current rabies vaccination, and a document confirming your pet has been microchipped. If you have any problems obtaining these documents please contact us to discuss your options. 
 
Please note the cost of issuing the AHC is £178 for registered clients for a single pet, each subsequent pet added to the AHC is charged at £85 per pet. (New clients registering to have an AHC will be charged at £211 for their first pet).

TAPEWORM TREATMENT 
Tapeworm treatment is required when travelling with dogs from Great Britain to EU listed tapeworm free countries including Northern Ireland, Malta, Republic of Ireland, and Finland. These dogs must be treated for tapeworm not more than 120 hours (5 days) and not less than 24 hours before time of entry into one of these countries.
